Gordano Valley PCN

What is a PCN?

Since the NHS was created in 1948, the population has grown and people are living longer. Many people are living with long term conditions such as diabetes and heart disease or suffer with mental health issues and may need to access their local health services more often.

To meet these needs, GP practices are working together with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas in groups of practices known as primary care networks (PCNs).

PCNs build on existing primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care for people close to home. Clinicians describe this as a change from reactively providing appointments to proactively caring for the people and communities they serve.

Each of the 1,250 PCNs across England are based on GP registered patient lists, typically serving natural communities of between 30,000 to 50,000 people (with some flexibility). They are small enough to provide the personal care valued by both people and GPs, but large enough to have impact and economies of scale through better collaboration between GP practices and others in the local health and social care system.

New MSK Website

We are really pleased to announce that the BNSSG MSK Website, My Joint Health Hub, is now live! https://myjointhealthhub.bnssg.nhs.uk/

The website covers self-help options for a range of MSK conditions, including information about ESCAPE-pain courses, the getUbetter app and shared decision making charts as well as local service information for smoking cessation, weight loss and physical activity.

The website is mobile friendly, with translation readily available and pages can be individually printed in a simple format for patients who do not have access to the internet

Cervical Screening Awareness Week, 19th-24th June

Going for your cervical screening can be difficult. Everyone has a different experience, and at completely different stages of life. Cervical Screening Awareness Week is about sharing experiences of screening and the ways in which your screening can feel just a little bit easier. There are stories shared by many different people, getting screened for different reasons, which should help to put your mind at ease and feel better. Getting your cervical screening is very important, so having tips and tricks that can make it easier is definitely worth it. We are pleased to announce that as a result of all of your feedback about the eConsult system, we will be introducing a new online consultation service this summer called Patient Triage. Many of you have told us that the eConsult system has too many questions and that you feel many of them are irrelevant. Our new online consultation service only has a total of 5 questions, meaning it will be far simpler and less time consuming for you to complete. You will be able to access this new online consultation service via our website, directly from the NHS app, or via our facebook page. Please see the below photo which shows you the questions it will ask. You are also able to attach a photo if you need to. You will be able to submit an online consultation at any time of the day, even when we are closed. We will still aim to respond to you by the end of the next working day, including the offer of an appointment if you need one. If you do not have access to the internet or find it difficult to use a computer or smartphone you can call our reception team instead.

Waiting for your hospital care

The Coronavirus pandemic has had a big impact on the NHS and hospitals are trying to resume routine services and keep patients safe, at the same time as continuing to treat Coronavirus cases. Hospitals are reviewing patients who have been waiting a long time, to understand their current position and treatment wishes, and to help them prioritise those in most urgent need. They are doing their utmost to ensure you get the treatment you require as soon as possible.

Support

All NHS organisations have a Patient Advice and Liaison Service (PALS) which is there to provide support and advice for patients. The PALS service for the hospitals in our area are as follows:

Guidance for Patients sending in Photographs
  • If possible, another person should take the photo for you.
  • Ensure you are taking a photo in a well-lit area (ideally with natural light) and avoid shadows.
  • Your photo should ideally be taken on a plain background.
  • Make sure you take a photo that is in focus (keep the camera far enough away from the site until the image focuses).
  • Please try to hold still or rest your limbs on furniture to keep it still.
  • If you are taking a photo of something on your arm/leg, your photo should show both arms/legs to allow for a comparison to the other arm/leg.
  • Take AT LEAST  2 photos: One close up and one showing the wider area.
  • Ideally you should take lots of photos and send us the best ones.
  • Take some photos from different directions.
